Background information:
On October 2, 1990, the Nobel Prize Committee announced at the Karolinska Institute in Stockholm, Sweden that the 1990 Nobel Prize in Medicine (Physiology) would be awarded to Dr. Joseph E. Murray, a 71-year-old honorary professor of surgery at Harvard Medical School who has devoted more than 30 years of research on organ and tissue transplantation.
On December 23, 1954, Professor Joseph E. Murray successfully performed the world’s first allogeneic kidney transplant for a pair of identical twin brothers. The transplanted kidneys survived for 8 years. The operation was also regarded as marking the beginning of the modern organ transplant era.
Along with Professor Murray, 70-year-old Professor ED Thomas of the Hutchinson Cancer Research Center in Seattle, USA, also received the honor (first proved that bone marrow cells can be transplanted allogeneically).
In 1982, Zhongshan Hospital Affiliated to Fudan University successfully performed the first living relative-donor kidney transplant.
On January 21, 2016, the kidney transplantation team of Fudan Zhongshan Hospital performed kidney cancer resection and kidney transplantation for a mother and daughter at the same time. This was also the first living relative kidney transplantation after small kidney cancer was removed in the donor kidney in Shanghai.
The serious shortage of kidney sources is the most serious problem faced by patients waiting for kidney transplantation, and the “living relative kidney transplantation” of mutual assistance and self-help within the family is currently a relatively effective solution.
Today we are going to tell you a heartwarming story of “mother-daughter love” in kidney transplantation in our Fudan Zhongshan Hospital in this cold season.
Things in the world are not always smooth sailing. The daughter with uremia must undergo a kidney transplantation to regain her life. The brave mother was determined to donate one of her kidneys. As a result, a 2cm malignant tumor was found on the kidney during the preoperative examination… In addition, all the examination indicators of Ling’s mother met the requirements of a living healthy kidney donor.
Can the kidney transplantation between mother and daughter go smoothly?
Can a kidney that already has kidney cancer be donated to help a daughter with uremia?
Can Miss Ling regain a new life?
Deep love between mother and daughter, donating a kidney to save her daughter
The story starts with Miss Ling (pseudonym), who is over 30 years old. Due to the long-term hydronephrosis of the left solitary kidney, which has not been diagnosed and treated in time, the condition has developed to irreversible end-stage renal failure. Miss Ling can only rely on hemodialysis to survive. The discomfort and inconvenience of long-term hemodialysis make Miss Ling very painful.
After consulting and inquiring from many parties, Miss Ling’s mother came to Zhongshan Hospital Affiliated to Fudan University and found Professor Zhu Tongyu, a kidney transplant expert. She hopes to find a way to help her daughter live a normal life again. According to the relevant rules of “living relative kidney transplantation” of mutual assistance and self-help within the family, Miss Ling’s mother hopes to donate one of her kidneys to Xiao Ling.
Originally, this was just an ordinary case that happened in the kidney transplantation team of Fudan Zhongshan Hospital. However, during the preoperative comprehensive physical examination of Ling’s mother, the doctors found that Ling’s mother had a 2cm tumor on her right kidney, and it was very likely to be a malignant tumor (kidney cancer)!
- Can the tumor on Ling’s mother’s right kidney be removed and then the remaining right kidney be transplanted to Xiao Ling?
- Will the tumor on the transplanted right kidney recur after long-term use of immunosuppressants after surgery?
Experts step in and make careful arrangements
The kidney transplantation team of Fudan Zhongshan Hospital attaches great importance to Miss Ling’s case. Professor Zhu Tongyu, Chief Physician Rong Ruiming, and Chief Physician Xu Ming personally took the lead in leading the kidney transplantation team to make careful arrangements. They consulted a large number of domestic and foreign literature and found that there were nearly 100 similar successful cases abroad. The long-term follow-up results of these cases abroad confirmed that the risk of postoperative tumor recurrence in the transplanted kidney after the small renal cancer in the donor kidney was removed and used as the donor kidney for kidney transplantation surgery did not increase the risk of postoperative tumor recurrence in the transplanted kidney.
Generally speaking, due to the effect of rejection, a tumor of one person cannot grow in another person’s body, which also provides the necessary theoretical basis for kidney donation for renal cancer.
The doctors of the kidney transplantation team of Fudan University Zhongshan Hospital informed Miss Ling and her family in detail and objectively about the long-term effects and risks that this kidney transplantation surgery may face… After fully understanding the situation, Miss Ling and her family expressed their willingness to accept the kidney transplantation surgery after the right kidney tumor of Ling’s mother was removed.
After obtaining the approval of the hospital ethics committee, Professor Zhu Tongyu, Chief Physician Rong Ruiming, and Chief Physician Xu Ming of the kidney transplantation team of Fudan University Zhongshan Hospital personally took action and made a detailed and thorough deployment of the surgical process before the operation to ensure the success of this “special” living relative kidney transplantation.
Racing against time, two operations were carried out at the same time
On January 21, 2016, Miss Ling’s kidney transplantation surgery and Ling’s mother’s right kidney tumor removal surgery started at the same time in the adjacent operating rooms of Fudan University Zhongshan Hospital.
Chief Physician Xu Ming first performed a right kidney removal operation through a small lumbar incision for Ling’s mother. Because the vein of the right kidney is short, in order to retain a longer renal vein to facilitate the kidney transplantation operation, after the right kidney was successfully removed, the right kidney was immediately perfused, the tumor was removed, and the wound was sutured.
The removed right kidney of Ling’s mother was immediately sent to the operating room next door, where Professor Zhu Tongyu personally performed a kidney transplant operation on Ling. When the anastomosed blood vessels were opened, a large amount of urine immediately flowed out of the ureter.
During the kidney transplantation operation, the intraoperative frozen pathology report confirmed that the tumor removed was clear cell carcinoma (malignant kidney tumor).
Mutual help and self-help within the family Living relative kidney transplantation = a new life with love
Ling’s new kidney started to work, and her beautiful new life also opened a new chapter!
The kidney transplantation team of Fudan Affiliated Zhongshan Hospital will closely monitor the condition of Ling’s transplanted kidney (outpatient follow-up).
Organ transplantation brings life options to more patients, but the scarcity of donor kidneys is a hurdle that patients waiting for transplant surgery cannot overcome. In the process of relying on dialysis to maintain life day after day and year after year, if living relatives can be used for mutual assistance and self-help within the family, it is a relatively easy option to implement.
The hope of a new life requires love and warmth.
The kidney transplantation team of Fudan University Zhongshan Hospital sincerely wishes more uremia patients to have a new life as soon as possible!
